Intuitive Surgical’s Q3 2026 earnings showed a strong quarter but included an antitrust settlement and a narrowed da Vinci procedure-growth outlook, introducing near-term financial and regulatory uncertainty after the company’s Oct. 20, 2026 earnings release.

Quarterly Results and Settlement Impact

Intuitive Surgical (ISRG) reported third-quarter revenue of $3.05 billion for the quarter ended Sept. 30, 2026, a 22% increase from $2.51 billion a year earlier. Worldwide procedures using the company’s da Vinci and Ion systems rose about 15% year over year, with da Vinci procedures up 14% and Ion procedures up 31%. Instruments-and-accessories revenue reached $1.80 billion, up 18% from the prior year. Systems revenue increased to $769.1 million from $590.4 million, while da Vinci system placements rose to 546 from 427.

Adjusted net income was approximately $1.05 billion, or $3.00 per diluted share, compared with $860 million, or $2.40 per diluted share, in Q3 2025. Recurring revenue accounted for about 84% of sales. The company attributed growth to higher procedure volume, increased operating-lease revenue, more system placements, and an expanded installed base.

On a GAAP basis, net income attributable to Intuitive was $523 million, or $1.49 per diluted share, down from $704.4 million, or $1.95 a year earlier. The decline reflected a $450 million pre-tax charge recorded in selling, general, and administrative expenses related to an antitrust settlement. The after-tax effect was $347 million, or $0.99 per diluted share.

The settlement followed mediation and includes a one-time payment and mutually agreed injunctive relief without any admission of liability or wrongdoing. The company said, "The settlement remains subject to preliminary and final Court approval."

Revised Procedure Growth Outlook and Pricing Actions

Intuitive narrowed its full-year 2026 worldwide da Vinci procedure-growth guidance to approximately 14%–15%, with management expecting results near the midpoint. This replaces the prior range of 13.5%–15.5%. The guidance applies specifically to da Vinci procedures for calendar year 2026. Management cited potential impacts from changes to Affordable Care Act premium subsidies, patient behavior, tender volumes, and competitive intensity in China.

Planned pricing and product-life adjustments are expected to reduce da Vinci instruments-and-accessories revenue per procedure by about 3%–5% on a mix-constant basis once fully implemented. These steps include extending the useful lives of selected EndoWrist instruments and volume-based pricing, anticipated to begin in the first half of 2027.

Adjusted operating expenses are projected to grow 11%–13% for 2026.

The earnings release was furnished as Exhibit 99.1 in an SEC Form 8-K filed Oct. 20, 2026. The company updated its investor-relations materials on Oct. 21, 2026.

"The settlement remains subject to preliminary and final Court approval," the company said in its earnings release.
