UnitedHealth Group’s third-quarter 2026 earnings on Oct. 13 outpaced expectations, prompting the company to raise its full-year profit outlook. Management cited easing medical-cost pressures and early gains from value-based care and artificial intelligence, factors traders flagged as potential sources of volatility.

Quarter Results and Guidance

UnitedHealth reported third-quarter net income of $4.11 billion, or $4.56 per diluted share, compared with $2.35 billion, or $2.59 per share, a year earlier. The company’s medical cost ratio (MCR)—medical expenses divided by premium revenue—was 88.4%. Analysts had expected adjusted earnings of roughly $4.12 to $4.15 per share and revenue near $111.3 billion to $111.5 billion.

Before the quarter, UnitedHealth’s full-year adjusted earnings guidance ranged from $19.50 to $20.00 per share, with an MCR target of 88.1%, plus or minus 25 basis points. The company also projected operating cash flow of about $24 billion and planned share repurchases of at least $5 billion.

The company raised its full-year profit outlook following the quarter, though the revised range and accounting basis were not disclosed. Chief Financial Officer Wayne DeVeydt said investments in value-based care and artificial intelligence were beginning to contribute to results. Analysts have identified medical-cost management, Optum Health profitability, Medicare Advantage enrollment, and Medicare Star Ratings as key factors for margin recovery.

The stronger-than-expected quarter and raised guidance have shifted near-term expectations for UnitedHealth. Investors are now focused on whether easing cost pressures and early returns from value-based care and AI initiatives can sustain a broader margin improvement.
