CVS Health Corporation reported second-quarter 2026 earnings on Aug. 5, 2026, that exceeded expectations as revenue and profit improved. The company raised full-year EPS and operating-cash-flow guidance while signaling caution about elevated cost trends and potential macroeconomic headwinds.

Quarter Results and Guidance

For the three months ended June 30, 2026, CVS reported consolidated revenue of $106.1 billion, up 7.3% year over year. GAAP diluted EPS was $2.31, and adjusted EPS was $2.58, both surpassing consensus estimates. Net income nearly tripled to about $3.0 billion. Year-to-date cash flow from operations reached $10.6 billion.

Operating income roughly doubled to $4.7 billion, aided by the absence of legacy litigation charges, while adjusted operating income rose 35% to $5.16 billion. Adjusted operating margin expanded 100 basis points to 4.9%, reflecting lower operating expenses.

The company raised its full-year 2026 guidance, increasing GAAP diluted EPS to $6.84–$7.04, adjusted EPS to $7.90–$8.10, and operating cash flow to at least $11.5 billion. It now expects revenue of at least $414 billion, attributing the upgrade to gains in its Health Care Benefits and Pharmacy & Consumer Wellness segments while maintaining a cautious outlook given ongoing cost pressures.

Segment Performance and Margins

The Health Care Benefits segment, which includes Aetna insurance, generated $37.5 billion in revenue, up 3.5% year over year. Adjusted operating income surged 85.5% to $2.4 billion, driven by lower-than-expected medical costs and bonus payments for highly rated government health plans. CVS spent a smaller share of premium revenue on medical expenses than anticipated, contributing to margin recovery despite a decline in individual exchange business revenue.

Health Services reported revenue of $51.8 billion, an 11.5% increase, with adjusted operating income near $1.7 billion, about 10% higher year over year. Growth reflected improved purchasing economics and a more profitable pharmacy drug mix, partly offset by ongoing client price improvements.

Pharmacy & Consumer Wellness posted $33.8 billion in revenue, roughly flat year over year, with adjusted operating income rising 10.2% to about $1.5 billion. Results benefited from a more profitable pharmacy drug mix, brand inflation, higher prescription volume, and contributions from prior Rite Aid asset acquisitions.

The quarter’s margin and cash-flow gains supported the company’s decision to raise its 2026 outlook, though management emphasized the outlook remains contingent on cost trends and broader economic conditions.

The company said it is "maintaining a cautious view for the remainder of the year" in light of continued elevated cost trends and potential macro headwinds.
